How do I schedule an Evaluation?

Call, text, or shoot me an email. I will respond within 24hr, and can often get you scheduled within the next day or so.

Do I need a physician referral?

Nope! Oregon is a direct access state. This means I am able to provide physical therapy without physician referral.

Do you accept my insurance?

I accept medicare, BCBS, and can take HSA/FSA cards, but I am out of network with other insurances. If you have out of network benefits I can help with getting you reimbursed. If you are unsure of if you have out of network benefits or not, let me know and I can check for you.
